Renewal applications must be postmarked on or prior to October 1st. There is no grace period  and the board is unable to make exceptions.

Your renewal applicationand fee must be post-marked by October 1, 2024. If continuing education is due, the Statement of Continuing Education Activities must be included with the application and fee. Renewal applications post-marked after October 1, 2024 will not be accepted and your license will expire. There are no exceptions and there is no grace period. You may not legally provide services in Wyoming without an active license. 

The Board will accept a check, cashier’s check, or money order. Please make payment out to State of Wyoming, Board of Physical Therapy and ensure your name and license number are on the payment. The application and fee may be mailed to: 2001 Capitol Avenue, Room 127, Cheyenne WY 82002.

Renewals can take up to three (3) weeks for processing which does not include mail time. Multiple calls and emails to the board office causes significant delays in processing times. If you would like to know the status of your renewal, please click the box below.
